Job Title: Hospital Discharge Support Worker
Contract Type: Fixed Term for 6 months
Hours: 5 days (36 hours) per week.(Must be available to work flexible shifts and hours)
Pay: £13.00 – £16.00 an hour
Location: Based at St Marys Hospital but will work at other hospital sites, care homes and regular trips into the community across the tri-borough (Westminster, Kensington and Chelsea, Hammersmith and Fulham).
You will be supporting the social workers with practical tasks to aid discharges from hospital i.e. copying keys, providing access for cleaning or equipment delivery, food/clothes shopping etc.
*Getting clothes, paperwork or other items necessary for discharge
*Getting keys cut
*Supporting with arranging blitz cleans
*Supporting with housing issues eg resolving repairs, applications for housing
*Supporting informal carers with practical tasks relating to discharge
*Liaising with handyperson services regarding minor adaptations
*Visit the homes of patients/services users as necessary to undertake tasks required before discharge
*Visit the homes of patients/services users as necessary to undertake tasks required after discharge
*Supporting with property protection duties as set out in the Care Act
*Ensuring pets are appropriately housed if they cannot be cared for
*Supporting and resolving problems related to utility suppliers
*Ensure patient/service users property – including patient information – is handled safely and securely
*Notify social workers or managers immediately of any serious concerns or issues affecting patients/service users.
*Carry out proper procedures for ad hoc charges (such as key cutting) to be paid and recharged to the relevant borough. Obtain authorisation for expenses from the Hospital Social Work team managers
*Produce and maintain all required documentation relating to intervention using approved paper forms and electronic systems.
*Input data into client information systems ensuring all information and data is accurate and up to date.
*Carry out responsibilities for safeguarding children and vulnerable adults and ensure awareness of specific duties in the role.
You will be required to have a valid DBS checked against adults and all mandatory training up to date within the last 12 months.
